Section 2. Qualification for Membership - The members of this Cooperative Union are open to all types of registered primary cooperatives and secondary cooperative federations.

Section 3. Requirements for Membership. The members must comply with the following requirements:  

1. Accomplished Membership Application Form

2. General Assembly Resolution or Board Resolution that resolved and confirmed the membership to the Union 

3. Board Resolution on authorized and alternate representatives to the union

4. Signed Membership Memorandum of Agreement

5. Signed Data Privacy Notice and Consent Form

6. Cooperative Profile

7. Photocopies of the following documents:

1. CDA Certificate of Registration

2. CDA Certificate of Compliance

3. BIR Certificate of Registration

4. Articles of Cooperation and By-Laws

5. Audited Financial Statement for the preceding year; and

6. Cooperative Annual Progress Report for the preceding year.

Section 4. Application for Membership. The applicant for membership shall file a duly accomplished form with the Board of Directors who shall act upon application within thirty (30) days from the date of filing. The Board of Directors shall devise a form for the purpose which shall, aside from the profile of the applicant cooperative, include the duties of a member to participate in all programs including but not limited to the remittance of the Cooperative Education and Training Fund (CETF) due to the Cooperative Union, annual fees, patronizing the projects, programs, researches, education, training, consultancy and management services of the Cooperative Union and, such other information as may be deemed necessary. 

The application form for members shall include an undertaking to uphold the By-laws, policies, guidelines, rules and regulations promulgated by the Board of Directors and the General Assembly. No application for membership shall be given due course if not accompanied by the membership fee of One Thousand Pesos (₱1,000.00) for primary cooperatives and Five Thousand Pesos (₱5,000.00) for secondary cooperative federations, which shall be refunded to the applicant cooperative in case of rejection.
